(For minors under the age of 18) I am the Parent/Legal Guardian of the applicant. I am fully aware that my Child has completed this application to be an Approved Volunteer (non-member) for the Stephanie Nicole Parze Foundation.  I understand that by signing this application  that my child is NOT a full fledged "Member" of the Foundation, however, must still fulfill the necessary requirements and abide by the Foundation's  Code of Conduct at all times.  My signature below indicates my consent to process this application. 

The Stephanie Nicole Parze Foundation (SNPF) in addition to email, will begin utilizing Text Messaging as a new method of communication effective January 1, 2025.  Text messages will, when necessary, be sent to the cell phone number we have listed in your membership profiles.  Text messages will not be sent out between the hours of 9:00 pm and 8:00 am, unless it is an emergency.  (This does not apply to any response team members).  Authorization to receive text messages is implied.  Text Messaging consent is part of your membership application.  by signing the application you give the SNPF implied consent to send such text messages.
